WELCOME TO THE 2016 SENIOR HOUSING DIRECTORY
In these pages you will find resources to help you plan for potential housing changes for you or a loved one. Whether
it’s moving to an independent living facility or straight to skilled nursing, this directory will get you started in the right direction.
Take time to investigate your options fully before they are needed and you will feel like you have greater choices and control over any changes.
If staying in your home is where
you want to be then consider updating your amenities to make it safe for you
to remain there. Simple things can make all the difference. Eliminate risks for falls; change door knobs to levers and drawer knobs to D-rings; make stairs safe with supports such as handrails or grab bars; be sure areas are well-lit especially
where floor heights change; create greater stability in the bathroom; and make sure entryways are safe. There’s much more but this is a start. See Senior Times of South Central Michigan each month for more. Socialization is critical as well
so be sure to get out and participate in area programs through the senior centers in each county, Senior Health Partners activities, or even take a stroll with
Miles for Memories.
A special thank you to all the organi- zations and businesses in this issue that helped to bring this comprehensive look at housing options to you.

CentraCare is part of the National PACE Program (Program of All-Inclusive Care for the Elderly) and serves to meet the health care needs of frail adults 55 or older living in Calhoun, Kalamazoo, and Eastern VanBuren Counties.
Services are coordinated by the CentraCare team to:
• Ensure safety in the home • Focus on quality medical care • Support caregivers • Promote wellness • Provide an opportunity for socialization
