They’ve continually expanded their services and reach to serve more of their neigh- bors at different points in their care journey. They introduced the first inpatient hospice residences in our region. They are pioneers of delivering end of life care at home. Their day services give families vital support and freedom. And they’ll keep striving to improve as medical science and technology advance... all while staying centered on your needs.
After decades of providing expert care as Hospice Care of Southwest Michigan, the orga- nization officially changed their brand name to Centrica Care Navigators, effective on November 1, 2021.

So why did they change their name? Quite simply, they had outgrown their original brand name because they provide much more than hos- pice care and wanted the flexibility to add more services in the future.
Their new name, Centrica Care Navigators, reflects their team’s expertise in guiding their clients and families throughout the entire journey of serious illness and delivering tailored care cen- tered on their needs and wishes.
